About Your Visit

Below please find information about our COVID-19 precautions, information for new and existing patients and how to have your medical records sent to our office.

COVID-19 Precautions

We are committed to the safety of our patients and staff and to minimizing the spread of COVID-19. All of our staff are fully-vaccinated against COVID-19.

All staff are required to wear face coverings at all times while inside our office. Face masks are strongly recommended for patients and visitors. Face coverings must fully cover the nose and mouth.

We regularly sanitize high-touch surfaces.

To uphold social distancing guidelines, patients are asked to attend appointments alone. Patients who require assistance, and patients who are minors, may bring one caregiver to their appointments.

If you or your caregiver are experiencing symptoms of COVID-19, we ask that you do not attend your appointment, regardless of your vaccination status. Instead, we ask that you call us to reschedule. Symptoms of COVID-19 include, but are not limited to:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Diarrhea
  • Nausea or vomiting

Patient Portal

Our secure online Patient Portal allows you to message our office, send and receive documents, request appointments, and view your chart notes.

Please contact our office to request a registration invitation.

New Patients

We look forward to meeting you! Please complete the following before your first visit with us:

  1. Complete your New Patient Packet (English | Spanish) and bring it to your appointment.
  2. Review our Notice of Privacy Practices (English | Spanish).
  3. Review our Financial Policy Form (EnglishSpanish).
  4. Please bring your photo ID and your insurance card(s) to your first visit.
  5. Please also bring a pair of sunglasses, as well as any eyeglasses and eye medications that you use.

Existing Patients

Thank you for being our patient. At your next visit, please let us know if your contact information, medical history, medication list, or insurance information has changed.

Insurances We Take

We are contracted with the following medical insurance payers and plans:

  • Medicare
  • Aetna PPO plans
  • Anthem Blue Cross PPO plans
  • Blue Shield PPO plans
  • Cigna PPO plans
  • CalOptima Community Network (Medi-cal/Medicaid) (NOTE: Your plan may require an authorized referral via your Primary Care Physician, NP, or PA in order to see us.)

If you do not have insurance, or if you have an insurance plan with which we are not contracted, we are glad to see you on a cash-pay basis.

We accept payment in the form of cash, check, credit card, and CareCredit.

Please note that we do not accept vision plans, including VSP.
